The dissertation is devoted to the problem of decrease of the incidence of obstetrical and perinatal complications amongst the pregnant women with myoma uteri by the development of the differentiated algorithms of clinical management on the pregravidar stage, during pregnancy and labour. The retrospective and prospective analysis of the risk factors for myoma occurrence during pregnancy was conducted and predictive criteria for placenta insufficiency and other complications of pregnancy as well as prenatal outcomes. There were investigated the chromosomal anomalies e.g. translocations, deletions and fragile sites which are diagnostic and prognostic indices for the patients with myomata. There was stated that pregnancy decourse in women with myomata increases the frequency of complications during pregnancy and labour as well as postpartum period. They development of obstetrical and prenatal complications is appear at the background of the disorders in the uterine-fetal-placenta blood circulation and morphological changes in the placenta and myometrium. The developed algorithm of clinical management of the pregnant women with myoma allowed to decrease the frequency of the obstetrical complications, improve perinatal outcomes and develop the optimum tactics of the dispensary surveillance on the pregravidary stage as well as during pregnancy and postpartum
