Essay

Disertation is dedicated to echocardiographic and clinical evalution of long term results of mitral stenosis treatment by percutaneous balloon valvuloplasty (PBMV) and closed transventricular mitral comissurotomy (CTMС). It was found in an early postoperative period after PBMV and after CTMС equally the clinical improvement more than 2 timer enlargement mitral valve area (MVA) and a more then 50 % decrease diastolic transvalvular pressure gradient (DTPG) occurred. After CTMС a more expressed increase of MVA and a more expressed decrease of DTPG in comparison with PBMV was seen. However remote result (5 years of observution) after PBMV appeared to be better - of mitral restenosis frequency was 12,5 % in comparison with 18,1 % after CTMС. The dynamics of morphological and functional changes in the heart and the dynamics of a clinical state of patients was similar to these data. The exception was the of left atrium sizes, which did not correlate neither with the MOSA nor with the DTPG. As the result of both types of mitral stenosis treatment normalization and prononnced decrease of systolic pulmonary artery pressure was observed even in cases with initial pulmonary artery hypertension of III degree.PBMV is the treatment of choice for the most grave patients with the V stage of MS. Its fulfillment in 47,4 % cases with the aim of either definite treatment or as preparatory step for radical intervention - mitral valve replacement.
