Essay

Object:thyroid (T) disease: papillary / follicular (P/F) carcinoma, diffuse nontoxic goiter (DNG).The aim of work:to develop differential pharmacotherapic approaches for thyrotropin inhibition in patients with P/F carcinoma of T, DNG and study a possibility of correction of suppressive therapy (ST) complications. To establish a specific effectiveness of synthesized compound 3,5,3’-triiodothyroacetic acid (TTAA) in experiment for further development of domestic drug.Methods of study and equipment:clinical, radioimmunological, chemiluminescent, instrumental, statistical; radioactive radiation counter Mark III, immune analyzer ACCESS, echograph TOSHIBA SSA-240A, hardware-software complex „Holter systems monitor ECG „DiaCard”.Theoretical and practical results: the appropriateness of conducting ST with P/F carcinoma of T, DNG, during that a possibility of reducing the supraphysiological l-thyroxine dose by 20% was established. During which a possibility of supraphysiologic thyroxine dose reduction for 20%was revealed. The dependence of autonomic regulation disturbances of heart rhythm on age and duration of L-thyroxine treatment was ascertained, a possibility of its improvement through adrenergic blockade was displayed. Novelty: differential approaches for conducting ST in patients with P/F carcinoma and DNG were developed. For the first time a prolongation of QTc interval was established and the character of autonomic regulation changes of heart rate depending on age and ST duration were specified. Necessity of cardioprotective therapy with bisoprolol during ST was substantiated. Specific effectiveness of synthesized compound TTAA was proven. Degree of introduction: results can be used in treatment of P/F carcinoma of T, DNG. Field of application:endocrinology, сlinical pharmacology.
