Grebennicov D. Optimization of anorectal abscesses surgical treatment

In dissertation the connection between the occurrence of anorectal abscess deep forms and presence of anaerobic flora in the environment of abscess is confirmed; on this basis the settings of local and general antibacterial therapy are specified - it should be based on the study of clinical and bacteriological features of pathogenic microorganism. The importance of wound planimetric research in the combination with observation of clinical signs and citologic picture the wound for documentary confirmation of the pararectal wound healing dynamics is scientifically grounded. On the background of qualitative and quantitative indices high efficiency of ozone-oxygen mixture local infiltration in the acceleration of pararectal wound healing has been proved for the first time. Surgical situations are defined at which surgical treatment by a two-stage method is expedient. The results of auto-fixative drainage application are firstly scientifically analyzed first and it is shown that this method of treatment allows preventing the relapse of anorectal abscesses in 95 % of cases. On the basis of morphological research it is proved, that the final forming of pararectal fistula develops in terms of 2 - 2,5 months, that allowed scientifically determinate the terms of repeated radical operations at the two-stage method of treatment.
