Kyyashko O. Regional intra-arterial chemotherapy of unresectable locally advanced rectal cancer

Object - an intra-arterial chemotherapy in treatment of unresectable locally advanced rectal cancer. Purpose - to promote efficiency of treatment of unresectable locally advanced rectal cancer. Methods - clinical, laboratory, endoskopic, X-ray, US, CT, statistical. Novelty - the first time developed original method of treatment of unresectable locally advanced rectal cancer, which consists of the cannulation of superior rectal artery, subperitoneal egestion of it on a skin as angiostome, implementation the regional intra-arterial chemotherapy. In the first time it is set by the mathematical model of proportional intensities of Koks, that the most influence on the survival of patients causes the method of treatment (p<0,001). Found out meaningful influence on the survival of patient of sex (p=0,021) and histological structure of tumour (p=0,019). Results - partial regression of tumour of rectum is marked in 52,1%±7,2% of experimental group and 18,0%±5,4% control, stabilizing in 33,3%±6,8% and 10,0%±4,2%, accordingly; complete regression of tumour is registered only for the 2,1% patients of experimental group (p<0,001). The median of survival in the group of research made 16,6 months, in a control group - 8,8 months (p<0,001). A method is inculcated in practice of the Donetsk regional antineoplastic center, city inter-district oncologic dispensary of Mariupol, Zhytomyr regional oncologic dispensary,Ternopil regional clinical oncologic dispensary.
