The dissertation is devoted to the problem of decrease of the incidence of obstetrical and perinatal complications amongst the pregnant women with obesity by the improving the complex differentiated alhorithms of clinical management on the pregravidar stage, during pregnancy, labor and in postpartum period. The prospective analysis of the risk factors for obesity and peculiarities of lipids, proteins and carbohydrates metabolism was conducted and predictive criteria for acute and chronic fetal distress, other complications of pregnancy as well as perinatal outcomes. There were investigated the chromosomal anomalies e.g. translocations, deletions and polimorphism which are diagnostic and prognostic indices for the patients with obesity. The developed algorithm of clinical management of the pregnant women with obesity with different nutrition correction allowed to decrease the frequency of the obstetrical complications, improve perinatal outcomes and develop the optimum tactics of the dispensary surveillance on the pregravidary stage as well as during pregnancy and postpartum
