Essay

Object is the sharp bleeding from chronic gastric and duodenum, sharp gastric ulcers, syndrome of Mellory-Weiss ulcers; A purpose is to improve the results of treatment of patients with the sharp bleeding from the overhead sections of digestive channel by development of the nosotropic grounded tactic of treatment and testimonies to the choice of method of endoscopic haemostasis; methods - clinical, laboratory, endoscopic, diagnostic and treatment, morphological, statistical; novelty - new scientific information which extends the understanding of mechanisms of sources of bleeding and his relapse from the overhead sections of digestive channel and on the basis of the improved tactic treatments considerably allows to improve quality of patients treatment; the computer models of prognostication of relapse of bleeding and algorithm of usage endoscopic methods of treatment depending on the state of bleeding source are developed, and also optimum combination of preparations, reducing a gastric secretion, doses and regimes of their usage, the morphological predictors of relapse of bleeding are determined, the criteria of choice of endoscopic methods of treatment are offered; results - optimum combination of endoscopic methods of treatment and antisecretory therapy is developed; this allowed to reduce frequency of relapse of bleeding for patients by chronic ulcers - from 17,5 to 5,1%, with sharp gastric ulcers - from 10,9 to 10,3 %, with the syndrome of Mellory-Weiss - from 7,4 to 3%; it is inculcated - in medical practice, in an educational process; sphere - medical practice.
