Essay

Object of the study: arterial hypertension (AH) and obesity (OB) with chronic heart failure (CHF). Purpose of the research: increase of efficiency of therapy of patients of AH with OB with CHF on the basis of finding out of role of hyperuricemia in the processes of remodeling of myocardium of left ventricle, violations of lipid metabolism and carbohydrate exchanges and determination of optimum chart of nosotropic treatment. Methods of the research: general clinical, anthropometric, immunoenzyme, instrumental, statistical ones. Theoretical and practical results: the analysis of research results allows a doctor-internist, cardiologist to take into account for the adequate estimation of weight and clinical flow of CHF and features of of remodeling of myocardium of left ventricle for patients with AH and obesity level of urinary acid and TNF-a. Presence of hyperuricemia for patients with AH with OB at CHF complicates the flow of disease, assists development of hypertrophy myocardium of LV and grounds the necessity of determination for the dynamics of supervision of level of urinary acid, indexes of AH, lipid metabolism, glucose and insulin the whey of blood. Setting of magnesium orotate in combination with the individually neat chart of treatment of AH, OB and CHF will be instrumental in rapid normalization of indexes of blood pressure, indexes of carbohydrate and lipid matabolism, that improves a clinical flow and prognosis of disease in same queue. Novelty: peculiarities of clinical course, intracardial hemodynamic, carbohydrate and lipid metabolism, the activation of antiphlogistic cytokine FNT-? during - hyperurikemy in patients with arterial hypertension with signs of chronic heart failure in the presence of obesity. It is shown that the hyperurikemy can be considered as early negative metabolic factor, which increases the risk of occurrence and/or progression of chronic heart failure in patients with arterial hypertension and obesity. Also, it’s shown the effectiveness of combination therapy of blood pressure, obesity, in particular, and with signs of chronic heart failure, with the additional designation of preparation of magnesium orotat, that quickly normalizes uric acid levels, indicators of blood pressure, heart rate, carbohydrate and lipid metabolism, which leads to a reduction in the risk of complications and significantly improves the course and prognosis of these diseases. Degree of introduction: results of the research were introduced in the work of Kharkiv city hospiyal № 27, the Kharkov Institute of Medical Radiology, Dergachi Central District Hospital and Dnepropetrovsk Central Hospital, as it is confirmed by acts on introduction. Results of the thesis were included into the syllabus for training students and interns specializing in Internal Medicine at Departments of Internal Medicine No. 2, Clinical Immunology and Allergology of Kharkiv National Medical University.
