The scientific work is devoted to actual problem of diagnosis, clinical flow and treatment of surgical grampositive and gramnegative sepsis in patients with suppurative-inflammatory processes of soft tissues. The results of examining and treatment of 226 patients with surgical sepsis were summarized. First group formed of 142 patients with suppurative-inflammatory processes of soft tissues, which had grampositive sepsis. Second group formed of 84 patients with suppurative-inflammatory processes of soft tissues, which had gramnegative sepsis. Difficult parameters of homeostasis was researched in all of it patients. Principal etiological factors to development of surgical grampositive and gramnegative sepsis were established. It were Staphylococcus and Enterococcus for grampositive sepsis, and E. coli and Ps. aeruginosa for gramnegative sepsis. Peculiarity of diagnosis and clinical flow of surgical grampositive and gramnegative sepsis was established for each of it forms of desease, even persymptoms. It made possible to take differential diagnosis of etiological determined forms of sepsis without additional outlay. Private working outs were introduction in program of treatment. It made possible reduce term of treatment from 51,2 to 32,6 days in first group and from 52,4 to 41,3 days in second group, and reduce mortality from 19,4 tо17,1 % in first group and from 24,4 tо 20,5 % in second group.
