Object of the research: Hypertension disease associated with obesity. Purpose of the research: To optimize the efficacy of myocardium left ventricle lesions diagnosis in patients with hypertension disease associated with abundant body mass and obesity. Methods of the research: General clinical, anthropometry, instrumental methods, morphological investigation, statistical analysis. Theoretical and practical results: The reasonability of application of index of LVMM to the height in m2,7 was proven, which allows more adequate assessment of LVMM with simultaneous increase of the body mass in patients with AH. It was shown that obesity and AH which are associated with increased activity of pro-inflammatory cytokines and dyslipidemia promote remodeling LVM in patients with AH. Therefore, the diagnosis and of the presence and degree of obesity based on BMI calculation and AO determining allows to choose pathogenetically substantiated strategy of treatment of these group of patients with AH. Novelty: For the first time, clinical and morphological findings about the influence of obesity, activation of pro-inflammatory cytokines on the state of the LVM in AH were compared. Association between abundant body mass and hypertrophy development and LVM dysfunction was established. As well as prevalence of concentric hypertrophy in hypertension patients. It is shown that the use of LVMMI allows more correct assessment of LVMH when compared with the traditional LVMMI. Interrelations between cytokine blood profile and structural functional rearrangement of the LVM in patients with AH associated with obesity. Comparison of different methodological approaches to AO diagnosis showed that only the patients with AO demonstrated significant increase of TNF-? and IL-6 activity. Degree of introduction: The findings of the research were introduced into the work of therapy department of Regional Clinical Hospital (Kharkiv), therapy departments of city clinical hospital No. 11 and City Centre for Diagnosis and treatment of arterial Hypertension. The findings of the research ware used in the teaching process of pathomorphology department (Kharkiv National Medical University) and department of general and clinical pathology of faculty of fundamental medicine (Kharkiv National University named after V.N. Karazin). Sphere of application: medicine, therapy
