In the dissertational work the presented results of diagnostics and surgical treatment of sick people of cholelithiasis which is complicated by obstruction of the terminal part of choledoch. There are new methods of diagnostics and ways of operations with application endoscopic technologies which will provide to transport not injury character of interventions and help faster liquidation of an inflammation in bile-excreting to system, normalisation of the basic indicators lithogenicity bile. Have been created tactical algorithm of diagnostics and surgical treatment cholelithiasis which is complicated by obstruction of the terminal part of choledoch. The proved advantages of offered tactics and ways of diagnostics and surgical treatment cholelithiasis which is complicated by obstruction of the terminal part of choledoch, in comparison with traditional, that appears in decrease general lethality with 22,9 to 2,5 %, postoperative lethality with 23,1 to 1,9 %, postoperative complications with 43,5 to 24,2 %, repeated operations with 29,2 to 2,9 %, relapses with 37,1 to 8,7 %, and also improved of a life, function normalization hepar.
