Object of the study: early postoperative functional ileus (FI). Purpose of the study: revealing of the part played by disturbances in some links of lipid metabolism, particularly processes of lipid peroxidation (LPO), in the development of early postoperative FI and, on the basis of obtained results, elaboration and pathogenetical grounding of prophylactic and medical measures, which contribute to an improvement of immediate results of its treatment. Methods of the investigation: general clinical, laboratory, morphological, instrumental (X-ray), functional (electroenterography), statistical ones. Theoretical and practical results: methods for prognosticating FI within an early postoperative period were devised; they are based on disturbances from the side of some links in lipid metabolism, namely an increased level of triglycerides and accumulation of LPO end products, malone one in particular. A pathogenetically grounded prophylactic tactics and a method of FI treatment using a complex of antioxidant drugs were devised. A method of complex treatment of patients with postoperative FI, using antioxidant drugs, was introduced into clinical practice; it contributes to an easier course of the postoperative period, a decreased rate of second surgical interventions on the abdominal organs, as well as makes it possible to reduce the in-patient stage of treatment. Novelty: the data of experiments and clinical examinations about the study of disturbances in some links of lipid metabolism, antioxidant defense system, LPO processes, as well as their influence on the mosphostructural and functional state of the intestines within an early postoperative period are important for a better understanding of FI pathogenesis in the patients, who underwent operations on their abdominal organs. A dependence of the postoperative FI course severity upon the state of the antioxidant-prooxidant system was studied. An algorithm was developed for prognosticating the appearance and course of postoperative FI. A method of complex treatment of patients with early postoperative FI, which includes correction of disturbances in the antioxidant defense of the organism, was devised. Degree of introduction: the basic propositions and conclusions of the thesis were introduced into the work of surgery departments of Kharkiv City Hospital No. 31 and Meshchaninov Hospital of First and Urgent Medical Aid (the City of Kharkiv).
