Bars'ka L. Early diagnostics and correction peculiarities of osteopenic disorders in children with chronic diseases of gastro-intestinal system.

Object of investigation Peculiarities of structional and functional state of bone tissue Goal of investigation: improving of early diagnostics and correction of osteopenic disorders in children with chronic diseases of gastro intestinal system Methods of investigation: clinical and anamnestic, biochemical, biophysical, immunological, analytical, informatic and statistical. Theoretical and practical results: Determination of systemic, treating and prophylactic methods of osteopenic disorders in children with chronic diseases of gastro intestinal system in children. On the base of study peculiarities of structional and functional state of bone tissue, rate and degree of osteopenic disorders in children with chronic diseases of gastro intestinal system the diagnostic and prognostic value of clinical, biochemical densitometry data was determined. Clinical course peculiarities of chronic gastrointestinal diseases in children and features of anamnesis data and physical examination were investigated. Peculiarities of mechanism of development of osteopenic disorders in children with chronic diseases of gastro intestinal system depending on disease course were revealed, that could help to improve early diagnostics of such disorders and ways of care. Algorithm of early diagnostics of osteopenic disorders in children with chronic diseases of gastro intestinal system was applied. The way of out patient care was determined, particularly need in correction of osteopenic disorders in children with chronic diseases of gastro intestinal system. Novelty: At first it was applied with the help of data of ultrasound densitometry the programme of early diagnostics of osteopenic disorders in children with chronic diseases of gastro intestinal system. It was proved, in children with inflammation of upper parts of digestive system state of mineral component of bone tissue changes, whereas in children with chronic non-specific nonulcerus colitis the state of tabecular component decreases more often. Results of bone remodelling markers in children with chronic gastrointestinal diseases showed markable decrease of bone formation and significant acceleration of bone resorption. These changes were associated with the chronic non-specific nonulcerus colitis and could be explained as a result of chronic inflammation and were verified by enlargement of interleukins IL-6 and TNF-?. The trace elements analysis showed significant decrease of essential trace elements and enlargement of toxic trace elements. The most markable changes were met in children with chronic non-specific nonulcerus colitis. Results of correction of osteopenic disorders will help to improve and to individualise the quality of medical care of children with gastrointestinal diseases.
