The dissertation is devoted studying of a problem of surgical treatment patients with сomplicated defects of a abdominal wall. Was made definition a conception "сomplicated defects of abdominal wall". Ascertainment of laws of occurrence of postoperative complications in patients with big and huge ventral hernias depending on a condition immunological reactivity an organism, conditions of tissue in a hernia's gate zone, bacteriological pollution, degrees of increasing of intra-abdominal pressure after operation. Is shown to carry out computer tomography researches in patients with the big and huge hernias against adiposity and to choose optimum surgical tactics. It is developed algorithm of medical-diagnostic actions and a choice to a way of a plasticity of the abdominal wall with the account of possibility of adaptation patient's organism before increasing of intra-peritoneal pressure after operation. Was improvement techniques allohernioplasty with using polipropilen artificial limbs which allowed to successfully liquidate widespread defects of a abdominal wall, to expand operability borders, to promote cosmetic appeal.
