Minukhin D. Surgical treatment of acute abscesses of lungs with the use of low–invasive technologies.

Object of the study: acute abscesses of lungs. Purpose of the study: to increase efficiency of complex treatment of patients with acute abscesses of lungs by means of development of low-invasive methods of surgical treatment in a combination with antiinflammatory and liposomal preparations. Methods of the investigation: clinical, laboratory, biochemical, microbiological, morphological, instrumental (radiological, bronchoscopic, ultrasonic, angiographic researches) and statistical. Theoretical and practical results: There was developed and experimentally proved the scheme of treatment of patients with acute abscesses of lungs which includes in active surgical tactics with the use of low-invasive technologies with application of a complex antiinflammatory and liposomal preparations. The developed method of treatment of acute abscesses of lungs has allowed to accelerate time of full healing of an abscess cavity, to reduce intensive care time, to reduce quantity of bed-days, to avoid synchronisation of purulent-destructive process and death at patients. There was developed and introduced in practical activities the device for a puncture and sanation of an abscess of a lung (patent of Ukraine № 33101 from 10.06.2008) which application has allowed to raise considerably efficiency of sanation of a cavity of an acute abscess of a lung and to reduce frequency of postoperative complications. Novelty: There was discovered character and degree histomorphological reactions of a tissue of a lung at introduction of antiinflammatory and liposomal preparations at patients with acute abscesses of lungs which were showed by reduction of residual of inflammatory process, reduction of expression and prevalence of a pneumosclerosis and sites of an atelectasis and conservation surfactant lining. There was discovered character and degree of functional reactions at introduction of antiinflammatory and liposomal preparations at patients with acute abscesses of lungs of such important system of a homeostasis of an organism, as oxidant-antioxidant. It has been established, that at application offered combination of antiinflammatory and liposomal preparations level of products of peroxidation of lipids was reliable decreased in comparison with application of traditional schemes of treatment.
