Dissertation is devoted the study of role of transforming growth factor 1 (TGF- 1), Р-selectin in modulation of intensity of cardiovascular remodeling processes in patients with essential hypertension in combination with type 2 diabetes and assessment the prospects of pharmacological intervention by the angiotensinconverting enzyme inhibitor and antagonist of angiotensin ІІ receptors. For the first time specified a role of TGF- 1 in realization of cardiovascular remodeling in patients with essential hypertension in combination with type 2 diabetes . It's set that increase of level of this cytokine in patients with essential hypertension in combination with type 2 diabetes to a great extent associate with the left ventricle mass index and indexes of blood pressure load of target organs and the most levels of TGF- 1 are observed at most prognostic unfavorable form of left ventricle geometry - concentric hypertrophy. Possibility of cardiovascular remodeling reversing is first well-proven due to blood TGF- 1 reduction under act of therapy by the angiotensinconverting enzyme inhibitor and antagonist of angiotensin ІІ receptors in patients with essential hypertension in combination with type 2 diabetes. Possibility of the use of assessment of blood TGF- 1 is shown for diagnostics of left ventricle hypertrophy and arterial vessels damage in patients with type 2 diabetes in combination with essential hypertension; expedience of determination of heart rate variability is grounded for the exposure of patients with an unfavorable prognosis in relation to cardiovascular complications at essential hypertension and type 2 diabetes combination; antihypertension strategy which is based on application of angiotensin ІІ antagonist candesartan has substantial advantages before treatment of angiotensin-converting enzyme inhibitor enalapril in connection with more optimum influence on the target organs damage.
