Truba Y. Surgical treatment of double outlet right ventricle (tetralogy type)

The study is devoted to improvement the results of diagnosis and surgical treatment of DORV (TOF type). Comparative analyze of dates of various diagnostic method performed. The new method of echocardiographic definition of DORV (TOF type) developed. This new method gave possibility to made accurate diagnosis preoperatively. Indication to performed pulmonary-arterial shunt was revised. In particular new minimal value of the end diastolic index of left ventricle and pulmonary artery index were define, that allow to performed biventricular correction. Efficiency of the systemic-pulmonary shunts and the most optimal period of their functioning were determined. The efficiency of one staged and two staged tactic of correction were analyzed comparatively. Definition of the most optimal period of systemic-pulmonary shunts functioning until definitive correction allow to avoid complications related to palliative procedures and to decrease time of heart functioning in pathologic condition. On the basis of study of perioperatively periods were define mortality and complication risk factors. Taking to account the obtained dates, the new algorithm of diagnostics and surgical treatment developed. As a result of executed research, effective system of diagnostics, operative management and postoperative treatment of DORV (TOF type) are optimized. The findings testify, that primary radical correction of DORV (TOF type) is the most rational to perform between one and three years of life.
