The dissertation is devoted to actual question of modern obstetrics - to establishment of role of vascular factor in forming of placenta insufficiency at diabetes mellitus, developments and introductions of complex of diagnostic and treatment-prophylaxis measures for such women. Exposed pathomorphology and histochemical features of placenta, which are substratum of placenta insufficiency at diabetes mellitus. It is shown that at diabetes mellitus placenta insufficiency which shows up violations of hormonal function of placenta from the early terms of pregnancy in heavy cases results in forming of microsomia. The disbalance vascular regulative systems is determine, which showed up violation of their equilibrium in the side of vasoconstriction, what considerable growth of ratio of endothelin/ nitric oxide and thromboxane/ prostacyclin specifies on. Estimation of specificity and sensitiveness of 44 indexes confirmed the leading role of endothelial dysfunction in pathogeny of development of microsomia. The complex of treatment-prophylaxis measures is improved for pregnant with diabetes mellitus and high risk of forming of heavy placenta insufficiency.
