Object of the study: arterial hypertension (AH) in combination with ulcerative disease of duodenum (UDD) in people, who took part in the elimination of consequences of the disaster at the Chernobyl Nuclear Power Plant.Рurpose of the research: an increase of the efficacy of treating patients with AH and UDD, who took part in the elimination of consequences of the disaster at the Chernobyl Nuclear Power Plant, on the basis of the use of pharmacological drugs and techniques of physiotherapy (ozokerite and direct voltage) with regard for peculiarities in the course of the diseases and effects of the medicinal preparations. Methods of the research: clinical and instrumental methods of investigation of the state of the cardiovascular system and gastrointestinal tract (oesophagogastroduodenoscopy, intragastral pH-metry and its daily monitoring), morphological, ultrastructural and statistical methods, single pharmacological samples. Theoretical and practical results: patients with UDD with underlying AH, who took part in the elimination of consequences of the disaster, revealed peculiarities in their clinical-functional state, the course of the diseases and treatment results, the above facts grounding the expediency of their treatment optimization. The direction and mechanisms of the haemodynamic influence of antiulcer drugs on the state of the acid-forming function of the stomach were revealed, thereby making it possible to administer them to patients having AH combined with UDD. Novelty: the thesis found out that AH in the participants in the elimination of consequences of the disaster left behind UDD, passed against a background of chronic insufficiency of the cerebral circulation and, in 79.0 % of the cases, of peripheral vessels. The above AH was characterized by a progressive course with frequent autonomic-vascular paroxysms, through it was with a less rate of the appearance of acute circulatory disturbances and a severe course of chronic cardiac insufficiency. It was found out that in 88.4 % of the participants in the elimination of consequences of the disaster UDD developed against a background of AH 10 years after the disaster and was characterized by the presence of atrophic and erosive gastroduodenitis, frequent relapses of asthenoneurotic manifestations, dystrophic and destructive changes in the subcellular structures of the gastric mucosa with the prevalence of catabolic processes over synthetic ones and disturbances in the intragastral microcirculation. Haemodynamic mechanisms of the effect of the antiulcer drugs, as well as of the vasoactive ones on the level of acidity of the gastric contents, were studied by the results of single samples. It was proved that the application of the developed method of pharmacophysiotherapy to the participants in the elimination of consequences of the disaster, who had AH in combination with UDD, improved the state of their cardiovascular system and gastrointestinal tract, did not cause any complications and reduced the rate of repeated hospitalizations by 37.5 % during 3 years of their follow-up.
