Object - patients with arterial hypertension (AH) and obesity. Aim - to improve the treatment effectiveness in patients with AH and concomitant obesity by means of investigation several data of endothelial function, lipide profile of blood and to establish the atorvastatin's and the ursodeoxycholic acid's usage. Methods - clinical, laboratorial, functional, statistical. New knowledge - it was first established specific influences of obesity on vascular endothelial dysfunction in patients with AH I and II stages depending on gender, it was proposed new approaches for pathogenetical treatment. It was discovered such pathophysiological mechanisms which caused vascular endothelial dysfunction in AH I and II stages in cases with concomitant obesity. Characteristic changes have been detected in vasodilatation L-arginin-NO chain presenting in patients with AH and obesity which were compared to the AH cases without obesity depending on body mass index. Results - atorvastatin being included in complex therapy of patients with AH and obesity causes reliable shifts in blood lipide profile but no influence on leptine level, thus, hypoleptinemic remedies are additionally needed. It has been evidenced that ursodeoxycholic acid assigned in complex therapy to the patients with AH and obesity had caused significant decrease of leptine concentration, therefore, ursodeoxycholic acid is allowed to be recommended into therapeutic standards in patients with AH and obesity for lowering the action of hyperleptinemiae which is an independent risk factor for appearances of cardio-vascular complications.
