Essay

It has been revealed that external irradiation irrespectively of internal one can predispose to development of differentiated thyroid cancer (DTC) in ChNPP accident survivors as a result of its promotion effect in dose range up to 10 cGy and through the DTC induction in case of 10 cGy and over. No linear relation between DTC latent period upon exposure and external radiation dose was found in 0.1-74.9 cGy dose range. The univariate analysis showed that the radioiodine ablation therapy, T-stage, histological type of cancer, thyrotropic hormone level and extent of surgery influenced the recurrence-free time. The multivariate analysis revealed that only T-stage and radioiodine therapy had an independent impact on recurrence-free period. The prognostic scoring system of DTC recurrence risk has been proposed. The update to long-term follow-up scheme for these patients was introduced. For the first time the factors that influance the second primary malignancies development in ChNPP survivors with DTC have been assessed.
