Essay

Object of study: Thyroid status in pregnant women. Aim of the study: based on a complex study of structural and functional changes in the thyroid of pregnant women with diffuse non toxic goiter dependent on the term of pregnancy, and peculiarities of pregnancy course, childbirth and prenatal development of fetus, to work out a tactics of pregnancy management in order to decrease the risk of obstetrical and perinatal pathology.Methods of investigations and devices: clinical, instrumental, laboratory, and statistical methods; USI device "SIM-7000" (Italy), photometer "FEK-M". Theoretic and practical results: a relationship has been established between thyroid size and functional state in women with diffuse toxic goiter and complications of pregnancy course, childbirth, and perinatal development of fetus. In this category of patients high levels of T4, total T3, and free fractions of thyroid hormones have been noted, compared to the group of pregnant women without thyroid pathology and control group. In pregnant women with diffuse non toxic goiter and in the group with risk of abortion, higher levels of thyroid hormones and lower TSH levels compared with control group were noted. It has been established that pregnant women of main group had low ioduria levels in first trimester, suggesting a relatively low iodine deficiency. The results of investigations will contribute to improving the efficacy of prognosis and prophylaxis of obstetric and perinatal pathology. Novelty: peculiarities of thyroid status have been established in pregnant women in different terms of pregnancy. Based on a combined analysis of thyroid functional state, pregnancy course, childbirth in pregnant women with diffuse toxic goiter, data have been obtained on the frequency and types of complications. Peculiarities of ioduria changes in pregnant women in different terms of pregnancy have been revealed. The rationale of prescription of iodine preparations to pregnant women with diffuse toxic goiter during pregnancy has been demonstrated. Degree of introduction: methodical guidelines for follow-up and management of pregnant women have been suggested. The results of investigation are being used in clinical practice of Odessa Regional Maternity and Consultation room No. 14. Sphere of application: endocrinology, obstetrics and gynecology.
