SUMMARY. Gavrish E.V. Endoskopic surgical treatment of patients with odontogenic maxillary sinusitis. - The manuscript. The dissertation for obtaining an academic degree of candidate of medical sciences on speciality 14.01.19 - Otorhinolaryngology. - Kolomyichenko Otolaryngology Institute of the Ukrainian Academy of Medical Sciences, Kyiv, 2010. The dissertation is devoted to the questions of endoskopic surgical treatment of patients with odontogenic maxillary sinusitis. Under a supervision there were 150 patients (74 men and 76 women) in age 16-76 years. The group of comparison was made by 36 patients with chronic rhinogenic maxillary and etmoid sinusitis, control group - 53 patients, which before in our clinic the Caldwell-Luc operation was executed concerning a odontogenic maxillary sinusitis. Periapical inflammatory processes and low location of maxillary sinus bottom in relation to the nose cavity bottom (on the average on 6,9±1,9 мм) are instrumental in development of disease. Changes in a ostiomeatal complex and parafunction of natural ostium of maxillary sinus are an important link in pathogenesis of chronic odontogenic inflammation, however there are they much rarer, than at rhinogenic maxllary sinusitis. Conservative treatment was successful at 44,7% patients with odontogenic maxillary sinusitis. Mikro- maxillary sinusotomia, executable through a canine fossula by a trocar, is pathogenetic interference for patients with odontogenic maxillary sinusitis, its efficiency on frequency of postsurgical complications, for certain exceeds efficiency of Caldwell-Luc maxillary sinusotomia. The original method of expansion of maxillary ostium is protected by the patent of Ukraine.
