The work is dedicated to research of anatomical, functional and morphological changes in urinary tracts in benign prostatic hyperplasia that is complicated with detrusor's function decompensation, their influence on the results of the surgical treatment and postoperative complications, improvement of surgical technique of transvesical prostatectomy and evaluation of long-term postoperative results. The work represents the analysis of complex evaluation and treatment of 217 patients with benign prostatic hyperplasia that is complicated with chronic urinary retention, neorogenic bladder dysfunction, ureterohydronephrosis and chonic kidney desease that underwent surgical treatment: 165(76%) - prostatectomy without bladder resection, 52(24%) - prostatectomy with megacyst or diverticula resection. The stages of anatomical functional and morphological changes in urinary organs in benign prostatic hyperplasia were analysed. The method of differentiation diagnostics of megauretre was worked out. The indications for resection of megacyst are scientifically explainedу. The improvements of surgical technique for benign prostate hyperplasia that is complicated with detrusor's function decompensation are worked out and the efficacy of urination recovery is evaluated.
