The thesis is devoted to the improvement of the results of the surgery treatment of the giant incisional ventral hernias, and particularly, to the reducing the frequency of recurrences and postoperative complications by optimizing the choice of the alloherniaplastic method with the monitoring the abdominal pressure, as well as developing new alloherniaplastic methods and the ways of postoperative complications prophylaxis. There were 236 patients with giant incisional ventral hernias at the age of 30-75 operated. The criteria of choosing the alloherniaplastic method in treating the giant incisional ventral hernias were developed by determining the width of abdominal rectus muscles and monitoring the abdominal pressure in modelling the herniaplastic during the operation. Also the ways of alloplastic with separating the anatomic components of abdominal wall and using mesh implants in combination with tissues of the hernia bag were worked out. Due to the developed ways of alloplastic and complications prophylaxis, there was the interabdominal hypertension only in 2 (0,8%) patients, pneumonia in 4 (1,7%). 2 (0,8%) patients died from pulmonary embolism. The were 9 (3,8%) with a seroma on the part of the wound, 6 (2,5%) with infiltration, and3 (1,3%) with festering. Hernia recurrences were observed in 3 (1,3%) of 225 examined ones within 5 years.
