Essay

The dissertation deals with actual problem of modern neonatology - special features of forming and prophylactic measures of bronchopulmonary dysplasia, wich is got by prematurely children with gestation age less than 32 weeks and birth weight less then 1500 grams. Spreading of bronchopulmonary dysplasia, perinatal factors of risk in bronchopulmonary dysplasia developing and clinical run features of the illness in case with deep prematurely children were determined. The role of inflammatory reaction and blood vesels development factors in bronchopulmonary dysplasia progress it's level of difficulty for prematurely children was ascertained (according to indicators of proinflammatory and antiinflammatory cytokines, vascular endothelial growth factor on the 5th-7th day of life and in dynamics in the 4th week of life). It was determined, that IL-6, IL-10, VEGF aren't only important predictors of bronchopulmonary dysplasia, but the markers of difficulty level of the illness. The morphologic and immunogistochemical peculiarities of lung tissue were investigated, the breaches in process of angiogenesis and alveologenesis were discovered during bronchopulmonary dysplasia forming. On the basis of research results the scheme of pathogenesis is offered. It was proved the efficiency of the early postnatal prescription of dexamethasone wich is to be lessen the level of difficulty of bronchopulmonary dysplasia (true decrease of AVL duration, duration of oxygen dependence) in comparison with the late prescription. During following-up research of the children having bronchopulmonary dysplasia it was determined the frequency of respiratory, heart complications, retarding in psychomotor and physical development, as well as a healthy children ratio.
