Semikoz O. Hygienic assessment of living conditions and optimizing the health of railroad workers in the industrial Donetsk region

Object-health of railwaymen which work in harmful industrial conditions. Purpose-hygienic substantiation and elaboration of measures on optimization of health protection of railwaymen which undergo intensive influence of industrial and ecological risk factors. Methods-hygienic, physiological, sociological, anthropometric and physiometric, clinical, economical, statistical. Novelty-for the first time classification of prenosological health states of railwaymen according to adaptative stages and functional criteria was substantiated; hygienic classification of ecological environment in open industrial zones including industrial risk factors for health state of railwaymen according to the level of valeological significance was substantiated, for the first time qualitative and quantitative parameters of structure of actual and healthy life-style of railwaymen were established; principles of core primary and secondary prevention of diseases were substantiated and formulated; for the first time a new scheme of closed technology of dispensary monitoring of railwaymen health state was substantiated; three-stage system of regional-district organization of family medicine was substantiated; for the first time technology and criteria of railwaymen health improvement in terms of basic and reinforcing course were developed and substantiated; organizational scheme of medical care financing of railwaymen by combined budget-insurance mechanisms was experimentally proved. Results of investigation were introduced into activity of railway treatment-and-prophylactic institutions: public institution “Railway clinical hospital on railway station Donetsk” of public establishment “Donetsk railway”; public institution “Railway clinical hospital on railway station Ilovaisk” of public enterprise “Donetsk railway”; public institution “Railway clinical hospital on railway station Krasnoarmeisk” of public enterprise “Donetsk railway”.
