Essay

The thesis is devoted to diagnostics improvement and cure efficiency enhancement on the basis of clinicopathogenic value of endotheliocyte dysfunction, leptinemia and lipid spectrum blood change examination of the patients with dysmetabolic cardiomyopathy (DMCMP) who have excessive weight and obesity. Women with DMCMP who have excessive body weight and obesity in the condition of perimenopause are proved to have a lot of complaints not connected to the considerable somatic alterations. It was determined that there is a significant percentage of glucose tolerance dysfunction with normal glucose indices fasting, which indicates the necessity of the background study of this index in this category of patients. The cardiovascular system changes have not been revealed at this stage, and endothelium-dependent and endothelium-nondependent vasodilatation is at the border between normal and pathologic. These women have a statistically confirmed leptin increase in blood serum, which is in direct correlation to bodymass index and the level of lipids in blood serum. The application of hormonal replacement therapy and phytoestrogens for patients with DMCMP who have excessive body weight and obesity has led to the increase of their standard of life, reduction of leptin content in blood serum and improvement of carbohydrate metabolism indices. There have been no negative effects on cardiovascular system function and lipid metabolism. The absence of therapy has caused the rise of blood pressure, negative alterations of triglycerides and high-density lipoproteins and leptin increase in blood composition.
