Tarasenko L. Surgical treatment of patients with residual cavities in chronic pleural empyema.

The main terms of the work contain data about improving the treatment results of chronic pleural empyema, based on the improving sanation of residual pleural cavity techniques, based indications for lung decortication or thoracomyoplastic operations, depending on the term of disease. The effectiveness of proposed methods of pleural cavity sanation based on ultrastructural changes of the cells of the aero-haematic barrier was proved. A device for thoracocentesis for thoracoscopic dynamic monitoring of the effectivness of the empyema cavity sanation was also proposed. According to the lung and pleura morphological studies, in patients with residual cavities in chronic pleural empyema with term of illlnes less than 90 days decortication with pleurectomy. In patients with term of illness more than 90 days the optimal surgical treatment is muscle plastic and intrapleural fragmentation thorascopy.
