Pak V. Optimization of surgical treatment of adhesive intestinal obstruction in the light of prognostic factors

Dissertation deals with the content of acute adhesive intestinal obstruction as a frequent manifestation of adhesive peritoneal disease. Proposed a new way of differential diagnosis with the objective quantative and rank evaluation of main diseases manifestation, which increased the effectiveness of early diagnosis and surgeon intervention. Evaluation of adhesive syndrome symptoms allowed to optimize surgical treatment of adhesive intestinal obstruction due to development and implementation of clinical practice diagnostic table matrix, that allows a high degree of probability point to the urgent need for surgical treatment. It was studied the character of immune response in patients with acute adhesive intestinal obstruction and ultrastructural changes of intestinal wall in dynamics. Proposed a new method of celiocentesis puncture as a safe and highly informative method for diagnosis of intestinal obstruction. Developed a method for quantifying X-ray symptom "Kloyber's bowl" in the dynamics that can differentiate adhesive peritoneal disease and adhesive intestinal obstruction and time to perform surgery. Application in clinical practice of new diagnostic techniques has allowed more quick diagnostic of intestinal obstruction and provide surgical treatment in favorable conditions. Number of complications was lower in the main group - 12.2%, while the comparison group - 29.5%. Mortality was 3.6% versus 11.4% in the comparison group.
