Ladonya S. A choice of surgical tactics of restoration of function of thumb in its traumatic damages.

Dissertational work is devoted to questions of a choice of optimum methods of surgical treatment of patients with traumatic damages of thumb. The analysis of structure of 132 patients with traumas of the thumb, passed hospitalization between 2004 and 2009, has shown the following: middle age has made 33.3 ± 7.8 years, men was 116 (87.8%), and women - 16 (12.2%). Quantity of household traumas 90 (68.24%), industrial - 34 patients (25.76%). On the basis of clinical data classification of methods of primary closing of defects of the thumb which together with ordering of its traumatic damages allows choosing an adequate method of operative treatment with minimization of the loss to donor sites has been developed. All methods have been divided in two groups: methods which allow keeping length of the thumb; methods which allow restoring its length. The offered surgical techniques with use of microsurgical technics have provided high-grade is functional-cosmetic parameters in 83.2%. The analysis of development of postoperative complications has shown advantages of closing of defects of tissue graftss with an axial blood-groove. The Developed and introduced ways of treatment have helped to raise the quantity of positive results, to avoid negative consequences, repeated surgical interventions and as much as possible to lower physical disability.
