Kostrovs'ka K. The efficacy of cataract surgical treatment in patients with diabetes mellitus

Object: cataract in case of diabetes mellitus. The purpose: to efficiency rising of cataract phacoemulsification in patients with diabetes mellitus (II type) by use of the new combined method, prevented postoperative exudative-inflammatory reaction, that consists of cataract phacoemulsification and intravitreal innjections of triamcinolon of acetonide. Methods: optical coherence tomography anterior segment, optical coherence tomography macular area of retina, cristallography of tear, visometry, biomicroscopy, ophthalmoscopy, statistical methods; optical coherence tomograph Visant OCT, optical coherence tomograph Stratus OCT. For the first time it was established that the crystallographic picture of tear with the salt's crystals occurrence in peripheric zone changes in case of diabetic macular edema that reflects the change of its protein's structure. For the first time established too that initial level of C-reactive protein is in blood of patients with diabetes mellitus and exudative-inflammatory reaction risk factors. The data have been specified that in such patients the intensifying of exudative-inflammatory reaction occured by increasing corneal edema, macular zone thickness and rising of C-reactive protein concentration in blood. The scientific data were dilated the fact of triamcinolon intravitreal introduction has limited system diffusion of the preparation for prevention of exudative-inflammatory reaction after phacoemulsification and provided long term prevention of the inflammation. The new method of diabetic macular edema diagnostics on the basis of tear crystallography and the new combined method of exudative-inflammatory reaction prevention after cataract phacoemulsification in patients with diabetes mellitus were offered.
