Essay

The role of endothelial dysfunction in forming of vascular complications were assessed. It was showed that for patients with diabetes the angyodystonic type of cerebral hemodynamics is typical. The results revealed that patients with diabetes had evidence of increased of the resistivity indecis and decreased of the velocity parameters, that confirmed a dysfunction of elastoplastic fatures of vessels as a demonstration of vascular remodeling that supported renal hemodynamics. It was showed that patients with diabetes have elevated oxidative modification products of proteins. As a resulat of oxidative stress an endothelial dysfunction developed and characterized by decreased of concentration nitic oxide and increased level of endothelin, vasopressin, von Willebrant factor plasma concentration. The inclusion of angiotensin-converting enzyme inhibitors in complex therapy from the second-year-duration of desease for the purpose of prevention of vascular complications was proved. It was demonstrated that required the combination of insulin analogues-glargin and glulisine from new-onset diabetes is recommended with the purpose to increase the efficiency of treatment.
