Essay

The dissertation is devoted to the study of the effectiveness of the early introduction of the preparation of the natural exogenous surfactant "Suzakrin" for prophylaxis and optimization of ALI/ARDS therapy in thermoinhalation injury (TII) combined with skin burns. It has been established that TII development is accompanied with decrease in superficially active properties of bronchoalveolar lavage. There have been revealed changes in the lipid composition of bronchoalveolar lavage in TII characterized by decreased content of phosphatidilcholine. It has been found out that the TII course is complicated with worse indices of gas exchange and respiratory compliance. Taking into account the results received, the methods of prophylaxis and early therapy of ALI/ARDS in TII with the preparation of natural exogenous surfactant "Suzakrin" have been elaborated. It is marked that replacement surfactant therapy corrects impairment of superficially active properties and lipid composition of bronchoalveolar lavage in TII. At that, the oxygenation index and respiratory compliance indices elevate. The duration of stay of patients with TII at the Intensive Care Unit becomes shorter, too.
