Essay

The dissertation presents a theoretic generalization and new solution for the scientific task, lying in restoring of natural fertility in men with acquired ejaculatory ducts obstruction by means of scientifically proven development of the aspects of their transurethral catheterization and balloon dilation, which improves the results of specialized medical aid in male infertility. Optimal terms of natural fertilization were established based on dynamics of effective ejaculation restoring to the development of obstruction ODE relapse. It was established that some patients with ODE have relative androgen deficiency, characterized by lowered androgen index as a consequence of increased concentration with normal testosterone level and moderately elevated FSH, caused by increasing age and earlier treated orchitis or epididimitis. It is an evidence of existence of a tendency to inhibiting testicles' function as a consequence of ODE, progression of which leads to serious alterations in spermatogenesis and, if the obstruction is not resolved, makes natural fertilization impossible. The irrationality of conservative treatment and recatheterization of the ejaculatory ducts is proven in patients with ODE relapse due to their low effectiveness and high failure and complication rate
