The dissertation is devoted to the analysis of interrelations between leptin, insulin, C-peptide and nitric oxide production system in patients with essential arterial hypertension, overweight and obesity. 118 hypertensive patients and 20 healthy persons were enrolled in the study. The elevation of leptin, insulin and C-peptide concentrations in hypertensives was revealed in comparison with healthy persons. In the group of hypertensive patients elevation of these humoral factors in blood was associated with overweight and obesity. The leptinemia is depended not only from the body mass but also from gender of patients. Generally, female have higher levels of leptin than males. The positive correlation between leptin, insulin and body mass index indicated pathogenetic relationship of these parameters in case of obesity and tissue resistance to regulatory hormones. The well-known markers of unfavorable prognosis of arterial hypertension - high systolic and pulse pressure, accelerated heart rate and concentric left ventricular hypertrophy - were associated with overweight, obesity, hyperinsulinemia and hyperleptinemia. The interrelations between levels of leptin, insulin and atherogenic lipids in blood plasma of patients with arterial hypertension and metabolic syndrome were analyzed. The leptin level in hypertensives significantly correlates with values of atherogenic dyslipidemia. This may confirm development of leptin resistance in the presence of atherosclerosis. According to obtained data the major determinants of inducible NO-syntase activity are central obesity and high blood pressure.
