Essay

The dissertation deals with the problem how to improve living liver donor’s perioperative management. Rational methods of anesthesia have been proposed based on the analysis of postoperative liver function tests in both right lobe and left lateral section donors. The advantages of gas or intravenous anesthesia combined with thoracic epidural block have been shown in different stages of donor surgical procedure. The guidelines for donor’s procedure anesthetic management have been developed. Blood-sparing strategy has been proposed based on preoperative autologous blood donation with bone marrow stimulation by recombinant erythropoietin. The autologous blood transfusion was performed on the end stage of surgical procedure. To minimize an intraoperative blood loss low-CVP conditions were created during hepatic parenchymal transection.
