The thesis is devoted to the actual question - the rise of efficiency of the relapse calcium-oxalate urolithiasis prevention by the way of a pathogenetically directed therapy. There were observed 94 patients suffering from RCOU, who gave their con-sent on participation in the research. The patients have been distributed into 3 groups: the patients of the first group were not administered antioxidants in the RCOU complex prophylaxis, the patients of the second group were adminis-tered -tocopherol acetate by the dose of 200 mg/day as a part of the complex prophylaxis, the patients of the third group were administered -tocopherol ace-tate by the dose of 600 mg/day as a part of the RCOU complex prophylaxis. The results of research testify that the most positive dynamics of oxalic acid metabolism indices was revealed in the patients of the third group, who had a relapse-free disease course during 3 years in 90% cases as compared with those patients who were prescribed low doses of antioxidants - 200 mg/day or with-out them. These patients had a relapse-free stone formation course - 78.6 and 72.2 % correspondingly. The strategy of RCOU pharmacological correction was scientifically grounded and developed; it lies in the basis of complex use of antioxidant pro-tection means, which are capable to normalize metabolism of oxalic acid and favors economical usage of endogenous antioxidants.Differentiated approach to choosing the dose of antioxidant preparation in the RCOU complex prophylaxis allows to promote its efficiency and decrease the cost of medical treatment
