Essay

The dissertation focuses on one of the topical obstetrical problems, which is to reduce the number of obstetric, cardiological and perinatal complications happening to pregnant women affected by cardiomyopathy by studying the condition of a fetal-placental unit, quality of life, psychological condition, the system of neurohumoral adaptation, and correlation between them and indicators of uterine-placental-fetal hemodynamics. These indicators were found to be significantly abnormal, in particular the functional condition of the fetal-placental unit was disturbed as testified by inhibited hormonal function; lower indicators of the fetal biophysical profile; deterioration of uterine-placental-fetal hemodynamics; and morphological and histological changes of the placenta. Criteria for early detection of fetal problems are suggested. The quality of life was found to badly deteriorate, individual and reactive anxiety to rise significantly. The role of stressogenic factors in the development of pregnancy complications and fetal problems was researched. A range of treatment and preventive measures aimed at correcting the detected neurohumoral and hormonal abnormalities, improving the quality of life and psychological condition is suggested to facilitate the reduction of complications happening to mothers and fetuses affected by cardiomyopathy.
