Object of research - a metastatic breast cancer (MBC) in the course of chemotherapy with the limited and unlimited term of treatment. A research objective: to improve results of system treatment of patients with MBC by a substantiation of optimum duration of chemotherapy, working out of indications and contra-indications to its continuation or the termination on the basis of definition of factors which demonstratively influence the general and recurrence-free a survival of patients. Research methods - clinical, laboratory, instrumental, morphological, immunohistochemical, statistical processing of results. Scientific novelty - is proved for the first time optimum duration of chemotherapy MBC on the basis of definition of the factors demonstratively influencing general and recurrence-free a survival of patients. It is established, that the best answer of a tumour to 24-week chemotherapy MBC will reach at localisation of metastasises in lungs and lymph nodes. For the first time feature of the answer of a tumour on long treatment at patients with the isolated metastasises in a liver is defined or at their combination to metastasises in lungs, necessity of chemotherapy without term restriction is proved. Results - the algorithm of decision-making on necessity of continuation or the termination of chemotherapy MBC taking into account indicators of the answer of the tumour, the estimated after 24, 32, 40 and 48 weeks from the treatment beginning, and localisation of the metastatic centres for the first time is developed. It is introduced in practice of clinical hospital N19 and versatile clinical hospital of N4 of Dnepropetrovsk, the Dnepropetrovsk regional clinic and the city oncological centre, the Krivorozhsky oncological clinic, the Cherkassk regional oncological clinic, in educational process of department of surgery with a course of oncology of the Zaporozhye state medical university, department of oncology and medical radiology of the Dnepropetrovsk state medical academy.
