Object - acute destructive pancreatitis complicated by pancreatogenic peritonitis; the objective - to study the specific characteristics of the pathogenesis, development and course of acute peritonitis as a complication of destructive methods of diagnostics, effective methods of treatment and prophylaxis; methods - general clinical laboratory, instrumental, microbiological and statistical-analitical; results - the leading components of the pathogenesis of pancreatogenic peritonitis, the sources and mechanisms of the formation of endotoxin, the rotes of their spread, the conditions and mechanisms of generalization, the role in these processes, of the enzymes of the pancreas has been investigated in the experiment, at has been corroborated on an experimental model that peritoneosorption with the use of sorbents with imparted antienzymatic and antimicrobial properties is an effective method of treating pancreatogenic peritonitis, the efficacy of using the elaborated methods of peritoneosorption in patients with pancreatogenic peritonitis has been evaluated and introduced into clinical practice, an algorithm of therapeutic management in patients with pancreatogenic peritonitis has been suggested, the result of using the new therapeutic approach are decrease in the indices of postoperative lethality and complications.
