Essay

Dissertation addresses an issue of improvement of differential approach to early prognostication of BPD and diagnostic of severity degree in premature newborns with severe respiratory disorders who underwent arti?cial pulmonary ventilation in neonatal period. Determined prevalence of bronchopulmonary dysplasia, factors of BPD pathogenesis are classi?ed subject to time as primary, triggering and secondary. Primary factors of pathogenesis are the following: congenital features of collagen synthesis and degradation, newborn gender, immaturity of lung tissue. Triggering cause of BPD development is an infection in the form of intrauterine pneumonia and APV which potentiate the in?ammatory process in the lungs. The main secondary factor of pathogenesis is cytokine TGF-?1 hyperproduction leading to formation of hyperplastic and ?brosing processes in pulmonary system and in pulmonary vessels intima. Increased serum TGF-?1 level on day 6-7 of neonatal period is also an important prognostic factor. In order to optimize early BPD diagnostic in newborns with high risk of lung tissue ?brosis, it is recommended to use sequential prognostic algorithms approved to be clinically effective which have been developed and implemented within the framework of conducted study.
