Essay

The dissertation deals with a problem of a suicidal behavior in patients with en-dogenous and exogenous depressions and with development of principles of differen-tial therapy and prevention. By means of an integrative clinical-psychopathological, psychometric, and pa-thopsychological investigation 120 patients with a suicidal behavior in endogenous and exogenous depressions (the main group) and 30 patients with depressive disorders without a suicidal behavior (the control group) were examined. It was found out that in patients with depression clinical-psychopathological predictors of a suicidal behavior were its somato-vegetative and anxious variants. At that anxious, melancholic, and somato-vegetative variants were predominant in pa-tients with endogenous depressions and somato-vegetative and asthenic variants were predominant in patients with exogenous depressions. In patients with endogenous and exogenous depressions it was determined a ty-pology of a suicidal behavior which included psychopathological, situational, and mixed types. The results suggested that 88.3 % of patients with endogenous depres-sions had a psychopathological type, 11.6 % of these patients had a mixed type of a suicidal behavior, and 100 % patients with exogenous depressions had a situational type of a suicidal behavior. On the base of defined clinical-psychopathological and pathopsychological re-gularities it was worked out a differential system of crisis therapy and prevention of a suicidal behavior in depression. This system includes pharmacotherapy and psycho-therapy. An approbation of the system of crisis therapy and prevention of a suicidal behavior in depression carried out by us suggested its higher therapeutic efficacy in comparison with methods used traditionally (recovery in 59.5 % and 35.1 %, p<0.05; a significant amelioration in 23.8 % and 21.6 %; amelioration in 7.1 % and 16.2 %; without changes in 9.5 % and 18.9 %; aggravation in 0 % and 8.1 %, respectively).
