Moshenska O. The ways of diagnosis and treatment optimization of acute ischemic insult on pre-hospital and early hospital stages.

An analysis of cases of verified acute ischemic insult, treated in the neurovascular department (department of neurosurgery № 2) Kyiv Municipal Clinical Emergency Hospital during 6 years (2004-2009), are presented in this thesis. The mortality from acute ischemic insult in the department is analysed, the characteristics of fatal and nonfatal structure (control) of acute ischemic insult is defined, the frequency and role of modified individual risk factors and their combinations in deaths from acute ischemic insult is determined. The state of cerebral insult prevention in outpatient phase is summarized. The timeliness and adequacy of emergency pre-hospital phase of insult, medical care for acute ischemic insult in the early hospital stage are summarized. The peculiarities of clinical manifestation and laboratory data of fatal acute ischemic insult, their dynamics, disease complications and causes of death in patients with acute ischemic insult in the hospital are detected. Based on these results, recommendations as to the ways of optimization of medical diagnosis and treatment process in acute ischemic insult in the pre-hospital and early hospital stages are made.
