Dissertation is devoted to investigation of levels of leukotriene C (LTC4), cytokines and markers of oxydative stress in patients with acute heart failure and optimization of treatment by adding to standart therapy of watersoluble form of Quercetin. At all clinical variants (acute decompensated heart failure (ADHF), hypertensive heart failure (HHF), acute left ventricle heart failure (ALHF)) it was the increase of the levels of LTC4, tumor necrosis factor-? (TNF-?), interleukin-6 (IL-6), C-reactive protein (CRP) the most expressed at ALHF and systolic myocardium disfunction. At patients with all clinical profiles the increase intensity of oxydative stress was marked. The greatest values of malone dialdehyde (MDA), diene conjugates (DC), ceruloplasmin (CP), catalase were marked at ALHF and lowered inotrope myocardium function, thus degree of processes of peroxidation prevailed over increase in formation of enzymes of antioxydant protection. At ALHF the highest indicators of remodeling a myocardium, low ejection fraction, mainly restrictive type of dyastolic dysfunction are revealed. At ADHF and HHF haemodynamics indicators, still were in limits of the standard norms, thus dyastolic dysfunction at HHF was characterised by type of infringement of a relaxation, at ADHF ? a combination of types of infringement of a relaxation and pseudo-normal.Treatment of acute heart failure by traditional therapy with inclusion of Quercetin in comparison with group of patients which received standard therapy without Quercetin, was accompanied by more expressed decrease in levels of TNF-?, IL-6; LTС4, MDA, DC, increase in activity of the CP.
