Essay

Subject of the study: patients with diabetes mellitus (DM) type 2 within 40-60 years old. Objective of the study: to research special changes in plasma coagulation and thrombocytic aggregation which patients with DM type 2 with vascular complications have and to improve complex therapy of DM on that ground. Methods of the study and devices: clinical, instrumental, biochemical, coagulological, statistical; optical aggregometer "Biola", automatic analyzer Autolyzer 20010D. Theoretical and practical results: increasing of procoagulative activity of endothelial function and thrombocytes and compensatory reactivity and further depression of anticoagulative part of haemostasis with advacing of diabetic angiopathies were determined. While using regression analyses for parameters of homeostasis connecting to albuminuria it was found out that the most influence on albuminuria level the haemostasis parameters had. It was set out that Von Willebrand factor and thrombocytic aggregation can be early markers of coagulation disorders with DM type 2. There were estimated short-timed and long-timed antithrombotic therapy with DM type 2. Novelty: for the first time it was found out the increasing of procoagulative activity of endothelial and thrombocytic parts of haemostasis on functional diabetic angiopathies and its dependance on duration of DM. Anticoagulation activity is elevated on initial stages of angiopathies and decreased along with progression of angiopathies. Prolonged antithrombotic therapy improved endothelial function along with securing their antithrombotic effect. Implementation stage: the results of study are used in clinical practice of Kyiv regional hospital, Kyiv endocrinological center, Kyiv clinic № 15. Application area: endocrinology, therapy.
