Panchenko N. Medical psychological monitoring of persons of young age with vegetative somatoformical disorders

In the dissertation with use of methods of demonstrative medicine and from positions of conceptually new approach to the complex account clinical psychopathologic and personal differences of patients vegetative somatoformical disorders vegetative somatoformical disorders on the basis of an estimation of risk and in uniform system of medical psychological monitoring develop multilevel system of consultation, psychocorrection, a psychoprophylaxis and diagnostics, and ways of an individualization of clinical reports are defined. Clinical psychopathologic features vegetative somatoformic disorders are taped and psychometric and personal differences of persons of young age with "isolated" and somatisated somatoformic disorders are defined. It is established that at patients with isolated vegetative somatoformical disorders the differential self-appraisal of a functional condition is characterized by primary depression of indicators of state of health and mood at less expressive depression of level of activity. At the same time at somatisated vegetative somatoformical disorders on all listed indicators authentic changes which are shown by their depression are taped. It is established appreciable self-restriction by patients with somatisated vegetative somatoformical disorders, and also features of their reaction to presence of these disorders, it forms a psychosomatic condition with high level both personal, and reactive uneasiness whereas at somatisated vegetative somatoformical disorders personal uneasiness authentically (р?0,001) is more that is a basis of formation of more serious disorders. At somatisated vegetative somatoformical disorders the emotional pressure and an unbalance of psychological processes and lability of nervous system that influences change of characterologic lines of the patient also is enlarged and leads to "bracing" in an internal picture of illness of proof psychosomatic disorders. The system of stratification of risk vegetative somatoformical disorders on the basis of studying of diagnostic value and prognostic value of factors of their formation is developed. At isolated vegetative somatoformical disorders are prognostically adverse: mood depression, high level sensitivity, low level of reactive uneasiness and activity. Mental working capacity of patients with vegetative somatoformical disorders is characterized by low adaptic reserves that is shown by ulternating attrition with fast development of fatigability and lability of the psycho-emotional status. Ways and structure of medical psychological monitoring (consultation, psychocorrection, a psychoprophylaxis, early diagnostics) vegetative somatoformical disorders are proved. By means of specially developed algorithms of clinical psychological diagnostics vegetative somatoformical disorders the problem of the differentiated approach to selection of standards of diagnostic and preventive activity is executed at isolated and somatisated vegetative somatoformical disorders. It in a screening-regimen allows to be defined concerning diagnostic and psychocorrectional tactics at the expense of use of the most informative criteria. The analysis of componental structure of efficiency of psychocorrection testifies to the differentiated character of influence that also is defined by clinical variant of vegetative somatoformical disorders. It has allowed to prove base stages of monitoring (screening-diagnostics-preventive maintenance) vegetative somatoformic disorders at young age.
