Essay

The dissertation presents theoretical generalization and new solving of a timely problem - improvement of diagnostics and treatment of cerebral dysfunction in patients with different stage of hepatic encephalopathy due to liver cirrhosis, based upon complex clinical-neurological, psychodiagnostic investigation, ultrasound dopplerography of cerebral vessels. Complex investigation revealed particular features of cerebral dysfunction such as vestibuloatactic syndrome, extrapyramidal signs, and their frequency and severity predetermine hepatic encephalopathy stage. Psychodiagnostic investigation of patients with hepatic encephalopathy due to liver cirrhosis revealed emotional disturbances (depression, anxiety), cognitive dysfunction (memory loss, disturbance of attention concentration). Reverse proportional correlation dependence between anxiety encephalopathy severities was determined. It was determined that in hepatic encephalopathy specific changed of cerebral hemodynamic develop. It was shown that in hepatic encephalopathy due to liver cirrhosis mostly vertebrobasilar system is impaired. It was proven that use of vasoactive agent cavinton (vinpocetine) increase the efficacy of treatment of hepatic encephalopathy. Positive influence of the medication upon cerebral hemodynamic and clinical data was determined. The proposed method of investigation of patients with hepatic encephalopathy due to liver cirrhosis increases the level of diagnostics of cerebral dysfunction, allows to reveal early impairment, to control efficacy of prophylaxis and treating activities.
