Malyshevskyi I. Treatment of patients of elderly and senile age with H. pylori negative acutely hemorrhagic by duodenal ulcers

Object - Helicobacter negative and Helicobacter positive acutely hemorrhagic duodenal ulcers; objective - to improve the outcomes of treating patients of elderly and senile age with Helicobacter negative acutely hemorrhagic duodenal ulcers via elaborating and introducing a new treatment approach; methods - general clinical, laboratory, instrumental, cytological, spectrometric and statisti-coanalytical; results - we have presented a new tehnique of surgical homeostasis of acutely hemorrhagic ulcer of the posterior wall of the duodenum, in case of its large size and elongated form, its higher efficacy having been also corroborated as compared with its closest analog, basing on the findings, concerning a study of the specific characteristics of the clinical course, the influence of local factors, the efficacy of endoscopic and injection therapy, thereby taking into account the presence or absence of Helicobacter infection, have mode it possible to develop a new algorithm of a surgical approach in case acutely hemorrhagic duodenal ulcer in patients of elderly and senile age; the prosed therapeutic approach based on the obtained experimental findings enables to reduce the rate of the onset of an early recurrence of bleeding by 8,1%, postoperative lethality - by 12,2%, and total lethality - by 13,6%.
