Object - various forms of peritonitis; goal - to improve the results of surgical treatment of patients with various forms of acute peritonitis by developing methods for early diagnosis of immune disorders and their assessment depending on the level of endotoxicosis and severity of peritonitis, rehabilitation of peritoneal cavity and using immunomodulatory therapy; the methods are general clinic, microbiological, immunological, morphological, biochemical and statistico-analytical; Findings -we have carried out a comprehensive assessment of nonspecific reactivity, cellular and humoral links of immunity, proteolytic and fibrinolytic activity of blood plasma, endotoxicosis, circulating immune complexes, morphological changes in immunocompetent organs (thymus, spleen and lymph nodes) in 64 patients with widespread development of peritonitis and their changes under the influence of the use of immunomodulatory therapy using blastomunil and immunofan, the algorithm of using immunomodulatory therapy in patients with disseminated peritonitis has been suggested, the method for rapid diagnosing of endotoxicosis level at peritonitis by means of Dunaliella viridis test kits, the method of individual selection of immunomodulating therapy in patients with peritonitis, a way of immunomodulating therapy and sanation of the peritoneal cavity with anolyte at acute disseminated peritonitis have been introduced which reduced the frequency of postoperative complications by 15 1% and the time of patients stay in hospital by 3.09 bed day.
